Register Nurse Supervisors can oversee your medication box and the CareTeam can be there at specific times during the day to ensure all medication is taken on time and as directed.

DISCHARGE PROGRAM

Our services help reduce the risk of re-hospitalization. The first 24 to 48 hours after a hospital or rehabilitative stay can be overwhelming. Upon discharge, the patient is often faced with the following issues:

How do I get home?

Who will be there to help me?

Who will pick up my prescriptions?

How will I get groceries?

Do I need to make a follow-up appointment?

Who will feed my pet?

Right at Home has created a discharge assistive plan for your patients going home after a hospital or a skilled nursing stay.

HOSPICE SUPPORT Right at Home offers our CareTeam 12 hours of advanced training to educated them on working with families whose loved one is at the end of their life. We supply care and

comfort to the client so the family can work through the

process of losing their loved one. The team is educated in

the role of personal care, positioning and hydration for end of life care.

Right at Home also offers hourly LPN Nursing services to

provide skilled needs to Hospice and Non Hospice

patients including but not limited to dispensing

medication, wound care and many others.

To start care, our RN's will meet the client to assess the situation and create a care plan as quickly as possible.

Hospice Support gives families the space to process what

is happening with their loved one and provides the hands on care so that the family can be family.

FLEXIBLE SCHEDULES

Right at Home is here to offer comfort whether it is one hour per visit or for 24/7 live in help. At Right at Home we believe you should have the service that you need to be safe and independent as little or as much as that might be. We have no daily or weekly minimums to meet your care and financial needs and budget. Let us be of service and create a custom plan so that we can continue to provide peace of mind knowing we are keeping things Right at Home.

If you are covered under a private insurance carrier weather for Workers Comp, Auto Accident, Disability and others, Right at Home is willing to work with your carrier to create a single contract agreement to serve your needs.

If you don't qualify for outside payment options Private Pay is the most commonly used. We accept ACH debits from checking accounts, credit cards and checks with a discount given for ACH payments.

Do you or your loved one have a Long-Term Care Policy in place? If so, Right at Home can work with you and the provider to pay for your home care services.

Are you a Veteran of the United States? If so, your benefits can be used as an option to pay for care. Ask us about eligibility.
