With the HSA you will need to pay out of pocket (with pre-tax HSA dollars) for prescription drugs until you meet your deductible. Once your out-of-pocket expenses reach the annual maximum, all plans will pay 100%. UNITED HEALTHCARE IS READY WHEN YOU ARE T ake advantage of United Healthcare’s dedicated Customer Service representatives, 24/7 online access to your benefits, and resourceful web tools to help you and your family live healthier lives. Go to your www.myuhc.com or www.welcometouhc.com PRESCRIPTION DRUG BENEFITS Whether you choose the PPO, POS or HDHP/HSA plans, prescription drug coverage is included. Your cost for a prescription depends on the type of drug you receive and where you obtain it. Under the HDHP/HSA plans, you pay the full cost, either out of pocket or with pre-tax HSA dollars, until you reach the deductible. Prescriptions can be filled at any UHC network pharmacy which includes many major chains and independent pharmacies. For your convenience, you can use the mail-order service to order maintenance prescriptions. Access pharmacy benefit for OptunRX thru www.myuhc.com for a more integrated experience. PRESCRIPTION BENEFIT PROGRAM – HOW TO SAVE MONEY: Prescription drugs can be quite expensive, especially if you have one or more maintenance prescriptions. There are several ways you can save on prescription drugs by understanding how the Buchanan Hauling prescription drug benefits work. The following are helpful hints: • Generic drugs are often the least expensive alternative. Ask your doctor if there is a generic option every time you are given a prescription. • For all maintenance medications – generic or brand – use the United Healthcare mail order program. Your prescription will be mailed directly to you.

7

2018 Buchanan Hauling Benefit Guide

• Use the pre-tax accounts to pay your out-of-pocket prescription expenses with tax-free money. You must enroll into an HSA for this benefit.

WHY CHOOSE GENERIC? Generic drugs are essentially the same as the brand-name drug in safety, strength, quality performance, and intended use. They also meet the same strict approval process by the Food and Drug Administration (FDA). Generic manufacturers don’t have expensive research and advertising costs which leads to lower pricing. 24/7 NURSELINE United Healthcare’s 24/7 NurseLine gives you access to qualified registered nurses. Nurses can help you understand how to handle a medical issue, decide the right level of care and answer your questions. To get answers any time of the day or night, call the number on your health plan ID card or log into www.myuhc.com. VIRTUAL DOCTOR VISITS Through UHC’s virtual provider network, you can connect with a doctor from your mobile device or computer 24/7. Doctors can diagnose and treat a wide range of non-emergency medical conditions and even write prescriptions. And, the cost of a Virtual Visit is typically lower than being treated at a doctor’s office, urgent care center or emergency room. Call the number on your medical plan ID card or connect through myuhc.com.

What is an HSA? An HSA (Health Savings Account) is a tax-free account you can use to pay for current and future medical expenses (even medical expenses during retirement). An HSA has triple tax benefits: • The money goes in tax-free • The money grows tax-free • Your withdrawals for qualified medical expenses – including any earnings – are tax-free. WHO’S ELIGIBLE? You’re eligible to enroll in an HSA if: • You enroll in a high deductible health plan such as the HSA plans. • Your only coverage is a high deductible health plan, and you have not signed up for Medicare coverage. If you’re covered under your spouse’s plan and that plan is not a high deductible health plan or your spouse contributes to a Health Care FSA, then you are NOT eligible to contribute to an HSA. You can contribute to your account – You decide how much to contribute up to the IRS limits below: Opening your HSA Account As the owner of the account, employees will need to set up an account if not already completed. The website is www.HSAauthority.com. The Buchanan Hauling code is 138537 . If you have any further questions, please contact Kimberley Brown in the HR department. If you enroll in the HSA plans, your account will need to be opened with HSA Authority no later than May 1st. to facilitate pre-tax payroll deductions.

PAY HEALTH CARE EXPENSES Each time you have a qualified expense, you decide whether to: • Pay out of your pocket and let your HSA grow, earning interest for future eligible expenses (e.g. medical expenses during retirement). • Use your HSA to pay for eligible medical expenses, such as your annual deductible and coinsurance. Your HSA can also help to pay for vision care, dental care, and prescription drugs. (For a complete list of eligible expenses, visit www.irs.gov.) ADVANTAGES OF AN HSA An HSA allows you to pay for health care expenses with pre- tax dollars. Money in an HSA can be invested much like 401(k) funds are invested. Unused money in an HSA account is not forfeited at the end of the year and is carried forward. Also, your HSA is yours to keep which means that you can take it with you if you retire.

Additional “Catch Up” if 55 or Older

You are not required to make HSA contributions, though it is a good idea to add to your account for tax savings and to help pay for medical expenses. You can contribute with pre-tax payroll deductions or contributions made directly to HSA Authority. These amounts are based on IRS limits.

Know the Difference between Preventive and Non-Preventive Care

Most plans cover preventive care services at 100%, however, you need to understand if your visit will be classified as preventive care or as diagnostic. Use the guide below to help you understand the difference.

Preventive Care

Non-Preventive Care

Mammogram – annually for women starting at age 40, or recommendation Colonoscopy – every 10 years starting at age 50, recommendation Annual physical/preventive care exam – includes height, weight, blood pressure Pap smear – once annually for women who are 18 years of age or older

Patient found lump in breast and doctor recommends mammogram to diagnose condition

Patient has unexplained weight loss and constipation. Afraid it’s colon cancer; schedules colonoscopy

Office visit due to fever and rash

Abnormal Pap smear; returns for second exam. This second exam would be considered non-preventive

Preventive care: Routine annual screenings to “prevent” illness or injury. Non-preventive care: If diagnosed with a condition, some screenings are often considered part of treatment. Be sure to talk to your doctor.

Services covered at 100% under Preventive Care Benefit: • Well child visits/immunizations • Routine adult physical exams • Mammograms • Routine gynecological exams including pap smear • DRE and PSA (Prostate Screening) • Colorectal cancer screening • Labs, pathology, chest x-ray, and EKG (when performed as preventive care) • Be sure your doctor codes the claim as “preventive.”

What is Healthcare Bluebook? Healthcare Bluebook has been selected by your employer as an added healthcare benefit to help you shop for care, compare facilities, and save money on healthcare services. Did you know that in- network prices for the same procedure can vary by over 500% depending on the facility you choose? Our web and mobile applications make it easy to save money on hundreds of the most common medical services and procedures by showing you the cost ranges in your area, and providing you with a selection of FAIR PRICE facilities. We also provide detailed information on the quality of common inpatient procedures (those that require a hospital stay). We enable you to easily identify and select a facility that has a high quality rating. What is the "FAIR PRICE?"

For services that include more than one provider, separate FAIR PRICE amounts may be shown for facility, physician and anesthesiologist. How do I find a FAIR PRICE? Finding a FAIR PRICE provider is easy using Healthcare Bluebook. Type in a search term or use the drop down menus to search for a particular service. Select the service you are looking for from the search results. Review the price range in your area and find the local FAIR PRICE for your service. Scroll down the page and review provider options for the service. Bluebook conveniently color codes all the provider options to make it easy for you to identify those providers with prices at or below the FAIR PRICE. Don't see a particular provider? You can always call and ask your provider for a price estimate based upon your insurance company rate. Compare the provider's price estimate to the Bluebook FAIR PRICE to determine whether or not the provider's price is in the fair range.

The FAIR PRICE is the amount you should reasonably expect a service or procedure to cost in your area. Bluebook FAIR PRICES are based on the actual amount paid on the claim, not the billed amount, and reflect the discounts that the health plan has negotiated with the facility.

Aren’t all hospitals good at all things? No, very few hospitals are good at all procedures. Some quality metrics use a single overall rating for each hospital. A single rating can mask important differences in the level of quality between clinical areas at the hospital. For example, a hospital can be among the highest performing facilities in the US for heart surgery, yet the same hospital can also be among the poorest performing facilities for joint replacement. How does Bluebook assign quality ratings?
